Step 1: Build the implementation team

Step 3: Conduct monitoring activities

The goal of this stage is to execute the intervention package designed and developed in the previous stage. The PI facilitator and various colleagues create and manage the implementation team, manage the overall implementation, and oversee organizational change processes. The most important outcome of this stage will be implemented interventions that close the performance gaps. As a result of the interventions, we should see the desired performance described earlier.

The outputs of this stage are:

  • The intervention team list
  • A written record of intervention arrangements made with cooperating agencies
  • An implementation plan
  • Interim reports on milestone achievements reflecting measures of intervention effectiveness
  • Completed interventions.
