Developed and pilot tested by IntraHealth, the new District Health Information System module streamlines surveillance and COVID-19 vaccination tracking in the country.

South Sudan has experienced numerous disease outbreaks before and after its independence in 2011, which have ranged from mild to severe. The South Sudan Ministry of Health continues to face several challenges managing and providing timely responses to emerging health threats.

An underlying reason for this is the country’s shortage of health workers at the national and subnational levels. This is compounded by an untrained health workforce and a lack of data for informed decision-making. Many health workers don’t have up-to-date training and cannot provide high-quality health care while information management processes can’t provide enough data for health workers to effectively manage disease outbreaks.

Over 68% of the health workers that manage data and information at national and subnational levels are volunteers. Over 72% of the counties transmitting pandemic and health management information system (HMIS) data have limited internet connectivity and unstable power. There is also a scarcity of functional hardware (computers and accessories) at the county and facility level to support timely data collection and use.

When COVID-19 began to spread, South Sudan invested in better surveillance and vaccine systems to help manage the country's pandemic response. IntraHealth International partnered with the Ministry of Health to provide financial and technical support to deploy the new District Health Information System 2 (DHIS2) Coronavirus Disease 2019 (COVID-19) surveillance, vaccination, and Adverse Events Following Immunization (AEFI) trackers. The trackers were designed to collect COVID-19 data in the country, inform surveillance and vaccination reporting, and facilitate travel during the pandemic.

"This will streamline COVID-19 vaccine data collection and reporting."

IntraHealth developed, customized, and deployed the DHIS2 vaccination tracker for pilot testing in 16 health facilities in Juba County, in partnership with the Ministry of Health and with support from the US Centers for Disease Control and Prevention. The tracker captures individual-level vaccination data and generates electronic vaccination certificates with quick response (QR) codes. It also allows for real-time monitoring of daily vaccinations in health facilities, which helps measure COVID-19 vaccine uptake in the country.

“With the launch of this new tracker, we’re focused on highlighting the key achievements associated with the system,” says Alfred Okiria, the project director for the Strengthening National Capacity for Integrated HIV/AIDS Health Data Collection, Use, and Dissemination in Support of an Evidence-based Response in South Sudan project. “The electronic vaccination certificate is particularly helpful and has allowed South Sudanese and non-South Sudanese citizens to travel during the pandemic.”

Working with the Health Information Systems Program (HISP) Tanzania, IntraHealth trained 87 data clerks and rolled out the system at the national and subnational levels.

South Sudan now has a national surveillance system and a vaccination tracker system.

“Now that we’ve launched the DHIS2 COVID-19 tracker, we’re looking forward to integrating it into health facilities around the country,” Okiria says. “This will streamline COVID-19 vaccine data collection and reporting and provides an excellent model for other countries who are looking to roll out similar digital health tools during the pandemic.”

Our strategy

To design and deploy the surveillance and vaccine trackers, IntraHealth used a variety of approaches:

Reviewed and updated data collection and reporting tools.
Customized, reviewed, tested, and deployed the DHIS2 system in collaboration with the Ministry of Health and critical partners (World Health Organization, ICAP, and Médecins Sans Frontières).
Trained data clerks from the six pillars of COVID-19 response (call center, rapid response team, laboratory, case management, home-based care, and point of entry for international arrivals).
Provided routine mentorship and coaching for the data management workforce and addressed emerging issues.
Procured 195 tablet computers to facilitate real-time data collection, reporting, and use at the national and subnational levels.
Provided monthly internet plans to data clerks to facilitate data uploads on the DHIS2 system.
Seconded 16 data clerks to selected facilities for data collection and reporting.
Printed and distributed data collection tools for paper-based data capture at facility and community levels.
Conducted routine supportive supervision visits and onsite technical support and mentorship to identify facilities that needed support.

Where we are now

South Sudan now has a national surveillance system and a vaccination tracker system, which replaced the old fragmented and parallel data reporting systems for emerging outbreaks. The systems can provide real-time data for decision-making and timely responses to emerging threats. The country has built health workers’ skills in using the incidence management system to respond to emergencies. It has adopted digitalization of data management and promoted a culture of data use. And the mentorship and coaching support have created a culture of continuous learning around data management to drive evidence-based decision-making on national pandemics within the country.

South Sudan's Health Workforce Is Better Equipped than Ever, Thanks to Investments in Strategic Information ihadmin2 January 13, 2023 November 03, 2022 Juba, South Sudan

For ten years, IntraHealth International’s Strengthening National Capacity for Integrated HIV/AIDS Health Data Collection, Use, and Dissemination in Support of Evidence Based Response project in South Sudan has helped transform the Ministry of Health’s data, policies, surveillance, reporting, and strategic plans. The results: a better-equipped health workforce and stronger health systems for South Sudan.

IntraHealth has been working in South Sudan since 2006 and began to focus on strategic information in 2012 with the Strengthening Systems, Capacity & Data Collection with a Focus on Strategic Information in South Sudan project. Together, the two strategic information projects:

Implemented a District Health Information System 2 (DHIS2) reporting system
Developed and updated national policies, guidelines, and strategic plans
Helped the Ministry of Health conduct surveys and surveillance and create a research and ethics review board
Strengthened monitoring and evaluation services in health facilities through field supervision
Founded the South Sudan Public Health Institute (SSPHI)
Designed and deployed a COVID-19 vaccination tracker and certificate.

The teams achieved these results for the country’s health system in the wake of conflict, insecurity, climate disasters, and numerous disease outbreaks, including the global COVID-19 pandemic. Adding to these challenges were South Sudan’s health workforce shortage of trained, skilled health workers. So IntraHealth began working with the Ministry of Health to gather and review data to inform their decision-making using DHIS2, a web-based open-source platform for data management and analysis.

Now, health officials can access data in real time.

“Data about health services in South Sudan were being sent by email at the county and state levels and were then transmitted via Excel spreadsheet,” says Alfred Okiria, the project director. “Now, health officials can access data in real time using DHIS2, which allows them to capture more data in a streamlined way.”

Over a two-year period (2020 – 2022) overall reporting improved from 43% to 77%. Reporting at PEPFAR-supported sites was at 100% by 2022. As reporting increased, the project team (with Ministry of Health and PEPFAR staff) continuously monitored data quality and trained 78 focal persons to conduct surveillance at 78 project sites.

IntraHealth trained over 1,300 data clerks along with 268 monitoring and evaluation officers and 650 health workers in 80 counties. Additionally, 25 field officers were trained during an eight-week program and seconded to 86 facility sites where they mentored and trained 1,540 health workers and helped community volunteers track clients who had been lost to care and bring them back.

“We used the granular site management (GSM) system—an intensive onsite program data review—and quality-improvement approaches to improve health workers’ performance, grow the HIV program, and improve the quality of HIV services,” Okiria says. “The field officers worked with their teams at the facility level, collected data daily and submitted them on a weekly basis to our staff who generated the weekly dashboard, reviewed performances and discussed them at GSM meetings, and created improvement plans.”

When COVID-19 began to spread, IntraHealth supported the setup of the incident management system and designed and deployed a DHIS2 COVID-19 tracker and vaccination tracker to support the COVID-19 data management. The project designed a verifiable digital COVID-19 vaccination certificate that over 3,500 clients used for travel across borders.

Turnaround time for reviews dropped from three-to-six months to three weeks.

At the institutional level, the team also worked with the Ministry of Health to overhaul the research and ethics review board. They established a secretariat, supported the hiring of a research director, and created an income-generating model. Turnaround time for reviews dropped from three-to-six months to three weeks.

The project also established a new public health institute and completed its strategic plan and monitoring and evaluation framework.

“It’s a key pillar for disease outbreak prevention, detection, and response,” Okiria says. “This is a historic project for the country and it’s something we will be part of for many years. It will help shape the future of global health in South Sudan.”

In June 2022, the South Sudan Public Health Institute graduated 42 people from the institute’s data management and leadership, management, and governance programs. It also supported the training of 99 field epidemiologists on the Field Epidemiology Training Program and 20 health workers on the incident management system.

South Sudan’s health workers are now more equipped than ever to provide high-quality services for their communities. That includes a much stronger data-collection and reporting system, diverse capacity-building programs such as fellowships and field supervision, intensive engagement with communities to increase use of health services, and collaboration and investment in program integration at all levels of the health system.

Together We Decide: An Interactive Game to Engage Men in Family Planning

More Information:	IntraHealth International partnered with ideas42 to improve postpartum contraceptive uptake in eastern Uganda through a behavioral design process. Our team developed a behaviorally informed intervention package that focuses on postpartum women, their male partners, and health workers. One component of the intervention is an interactive game, “Together We Decide,” which engages male partners of postpartum women. During this workshop, audience members will engage with game content by playing an abbreviated session of the game. During game play, the behavioral rationale behind some of the game cards and features will be explained to help participants think about its relevance for their own settings and programs. The workshop will conclude with a chance for audience reflection and questions and answers.

Health workers in 20 districts throughout Rwanda are making higher-quality health services related to COVID-19 available to more people in their communities, thanks to training through IntraHealth’s USAID-funded Ingobyi project. Together with the Rwanda Ministry of Health, we are improving the quality and accessibility of services related to family planning, maternal and newborn health, as well as malaria prevention and treatment.

“When COVID-19 began to spread in 2020, we had to pivot our preexisting plans and implementation strategies to make sure health workers had the right supplies and training they needed to keep communities safe,” says Samson Radeny, IntraHealth’s country director in Rwanda. “We used a variety of approaches to focus on both health worker response and community sensitization.”

And while the Rwandan government had solid outbreak-response structures in place to protect against the spread of Ebola from neighboring Democratic Republic of the Congo, they needed to quickly bolster and tailor these systems for the COVID-19 pandemic. So the Ingobyi team advised and provided technical assistance to the government to train the country’s health workers, build needed infrastructure, and communicate effectively about COVID-19. Together, we:

Trained 280 health workers on COVID-19 case management and care in 26 hospitals.
Partnered with 10 radio stations to disseminate messages about COVID-19, malaria, and maternal, newborn, and child health.
Trained 514 health workers at 25 supported district hospitals to accurately screen clients for noncommunicable diseases and link positive COVID-19 individuals to the national vaccination program.
Provided the Rwanda Ministry of Health with advisors during nationwide mass COVID-19 vaccination campaigns to help plan, coordinate, and supervise the national vaccination activities.
Renovated eight COVID-19 isolation facilities and equipped them with the needed items to support management of critically ill patients.
Repurposed the Ebola call center to serve as an emergency hotline for COVID-19 information and staffed the call center to provide information for the 84,000 calls per week during the first nationwide lockdown.
Procured 500+ pieces of equipment needed by health facilities including digital blood pressure machines, ICU beds, oxygen cylinders, and infusion pumps.
Trained 859 hospital staff on infection prevention and control. Of these, five clinical staff from each hospital were validated as mentors and in turn cascaded the same trainings to 325 health center managers, who in turn trained 3,378 health center staff.

During 2021, the second year of the COVID-19 pandemic, IntraHealth International reached 317,443 health workers in 42 countries, helping to make high-quality health care available to more people around the world. Our programs led to:

$33,399,555 in financial aid provided for student health workers through IntraHealth programs
2,524,424 COVID-19 vaccines distributed through IntraHealth programs
223,677 men provided with voluntary medical male circumcision to prevent HIV infection
2,264,216 messages sent to and from health workers via mHero, WhatsApp, or other digital health tools
898,190 individuals tested for HIV and received results
8,231,279 family planning users through IntraHealth programs
1,389,009 new users of contraception
504,185 children under 5 treated for malaria

Even before the pandemic, the World Health Organization estimated a shortfall of 18 million health workers by 2030. Now, 15 million more health workers are needed in low and lower-middle income countries alone to respond to the needs of the health system and achieve universal health coverage.

IntraHealth International reached 317,443 health workers in 42 countries.

“The difficulties many countries have faced in responding to COVID-19 and rapidly delivering vaccines, tests, treatment, and supportive care have shone a spotlight on vulnerabilities in health systems and workforces around the world,” says Polly Dunford, IntraHealth’s president and CEO. “But we know that by investing in and supporting health workers—including and especially community health workers, nurses, and midwives—we can make great strides in COVID-19 response and in achieving universal health coverage.”

In 2021, IntraHealth focused on expanding digital health tools, responding to global health security threats, and training and supporting local health organizations. We expanded our iHRIS software to 25 countries, who now use it to manage their health worker personnel records, trained 9,000+ health workers on COVID-19 response, and worked with 74 local organizations to help them prepare to receive funding from USAID and PEPFAR.

US Funding for Global Health Security Doesn’t Cover These Essential Systems for Health Workers—Yet kseaton March 28, 2022

Strong data systems allow countries to surge health workers during emergencies. New funding should cover this.

US funding for global health security has surged in the last few years, and now Congress has just approved a $510 million increase for fiscal year 2022, nearly doubling current funding. President Biden is proposing even greater increases for fiscal year 2023 in his new budget proposal, including a multilateral fund for global health security and pandemic preparedness. The US has already pledged $250 million to this proposed fund, which has a goal of $10 billion in annual funding from the global community.

The urgent question is this: How do we best use these funds?

The difficulties many countries have faced in responding to COVID-19 and rapidly delivering vaccines, tests, treatment, and supportive care have shone a spotlight on vulnerabilities in health systems and workforces around the world.

Historically, US global health security funding for the health workforce has focused on training field epidemiologists and preventing and detecting zoonotic diseases—valuable and necessary functions. The multilateral global health security agenda, which the US supports, has a similar emphasis for the workforce.

But high-level reviews of the lessons we’ve learned from COVID-19, such as the Independent Panel for Pandemic Preparedness and Response, have called for much broader investments in human resources to improve resilience.

We must expand our definition of global health security.

We must expand our definition of global health security to go beyond the traditional focus on disease surveillance capacity.

Frontline health workers—including community health workers—are every country's first defense when it comes to detecting, reporting, and responding to emerging threats and to building communities’ trust in their health systems. These workers need medical equipment, supplies, PPE, access to COVID-19 therapeutics, and solid, up-to-date data to do their jobs.

So our definition of global health security must include them. We can start by adding investments in the information and communication systems health workers need to do their jobs—the same systems that allow governments to make the most of their limited workforces by allocating them as efficiently as possible during emergencies.

I hear this consistently from IntraHealth International’s program leaders around the world.

“Clinical health workers play a major role in public health,” says Janet Muriuki, a physician from Kenya and our senior director for health workforce development. “We need to teach health managers how to use data for informed decision-making. This includes systems that ensure real-time and up-to-date data. All of which calls for a human resource information system with dashboards that can answer questions based on national and subnational needs.”

These systems—and the level of data literacy Muriuki describes—are paramount to our global health security.

During any emergency, governments must be able to surge their health workers to outbreak zones, where they’re needed most. These surges require careful planning to minimize impact on other health services. And they require real-time information on the location and skillsets of health workers, including the community health workers who are invaluable in spreading the word and helping keep primary care available during emergencies.

But what if a government has no idea where the country’s health workers already are, whether they’re trained to deal with emerging infectious diseases, or even how to reach them? Unfortunately, this is the case for many countries right now. Current US government funding for global health security does not help ministries of health know where their health workers are, what skills they currently possess, what further capacity they need, or how to best deploy them.

That’s a big problem.

Fortunately, there are human resources information systems out there—like IntraHealth’s iHRIS, which is free and open source—that can give decision-makers the data they need to surge health workers as part of a rapid response to any emergency. In fact, we worked with officials in Mali and Kenya during the beginning of the pandemic to use iHRIS data as part of predicting COVID-19 hotspots and making sure health workers were available when and where they were needed most.

Combined with an interoperable communication system such as mHero, governments can use these systems to not only surge health workers, but to track vaccination status among health workers, generate reminders about follow-up doses, monitor health worker performance and service delivery data, and more.

Health workers are at the very heart of global health security.

By harnessing the full potential of their data through interoperable health information systems, health officials can strategically deploy health workers where they’re needed most before and during pandemics, humanitarian emergencies, and other emerging threats.

“The agility with which countries were able to manage surge health workforce demands has been a key difference between successful and struggling responses,” the Independent Panel for Pandemic Preparedness and Response reported in their assessment of the COVID-19 response.

We should heed this assessment and recognize that health workers are at the very heart of global health security. When we envision a world prepared to respond to COVID-19 or the next pandemic, it must be one where governments have funding and personnel to keep them and their systems strong, keep their data up to date, and interpret them both every day and during emergencies.

Health Workers Matter for Global Health Security: Here’s Why kseaton March 16, 2022

Frontline health workers have been at the forefront of the COVID-19 pandemic. From providing care to critically ill clients and educating communities to supporting vaccination campaigns, they are crucial to global health security and pandemic preparedness. To be fully prepared, health workers need to be trained and supported with a robust human resources for health system. This panel of experts will explore how we can better prepare for future pandemics and increase our global health security by investing in and supporting health workers around the world.

Janet Muriuki, senior director of health workforce development for IntraHealth International, will moderate the event with opening remarks from Polly Dunford, president and CEO of IntraHealth International.
